Clinical and Histological Comparison of Pulp Anaesthesia and Local Diffusion after Periodontal Ligament Injection and Intrapapillary Infiltration Anaesthesia

چکیده
Background: Periodontal ligament injection (PDL) is used to achieve single tooth pulp anaesthesia; infiltrative, non-osseous intrapapillary anaesthesia (IPA) may be an alternative with a minor complication rate. Therefore, the aim of the study was an evaluation and comparison of PDL and IPA techniques via clinical human and animal histological methods. Methods: For clinical evaluation, 30 healthy volunteers were injected with both techniques in a split mouth design with 2 mandibular premolars on each side. Pain of injection (VAS) as well as an electric pulp testing via pulp vitality scanner was performed. Length of anaesthesia was measured. Two male sheep were injected with coloured anaesthetic solution in a split mouth design with 3 teeth on each side. Macroscopic surfaces of the non-decalcified bone parts, microscopic evaluation of the non-decalcified bone parts and histological sections with decalcified cut surfaces were examined. Results: Human clinical examinations could show no significant difference in injection pain; though, PDL resulted in a more profound and longer pulp anaesthesia (both p<0.0001) than IPA in all cases. In animal histological examinations, the local spread of anaesthetic agent into surrounding bone after PDL could be shown. In contrast, for IPA, mainly soft tissue diffusion was seen. Conclusion: According to clinical and histological data, profound bone diffusion – as to be seen in PDL seems to be the essential factor for successful single tooth pulp anaesthesia. *Corresponding author: Peer W. Supplementary routes to local anaesthesia.
The satisfactory provision of many dental treatments, particularly endodontics, relies on achieving excellent pain control. Unfortunately, the administration of a local anaesthetic solution does not always produce satisfactory anaesthesia of the dental pulp. This may be distressing for both patient and operator. Fortunately, failure of local anaesthetic injections can be overcome. This is often...
متن کاملInduction of general anaesthesia with intraosseous injection of thiopental in rabbits
The technique of intraosseous (IO) infusion has been reemerged as an alternative route of intravascular access for the delivery of fluids and some medications. Complications of IO infusion occur rarely and include fractures and osteomyelitis after long-term use of IO access. The purpose of this study was to compare the efficiency of induction of anaesthesia by a standard intravenous route and a...

A nulliparous non-descript mare was presented with a complaint of dystocia. The mare was recumbent and physical examination revealed that the animal was in shock. There was no straining and foetal forelimbs were visible outside the vulva. The foetus was dead as there was no pedal reflex. Vaginal examination revealed anterior presentation with dorso-sacral position and rigid lateral head deviati...
متن کاملTooth Regeneration with Stem Cell Sources
Introduction: During the last decade, advances in tissue engineering and stem cell-based tooth regeneration have provided realistic and attractive means of replacing lost or damaged teeth. The first adult stem cells isolated from dental tissues were dental pulp stem cells (DPSCs). When transplanted with hydroxyl apatite/tri calcium phosphate (HA/TCP) powder, they formed a dentin-like structure...
